A case manager is a form of integrated service at an individual level. The case manager coordinates the services for the user, especially for users facing complex and long-term needs such as people living with rare diseases. The case manager is able to provide individualised guidance to people’s needs and to reach out to local professionals, in order to complete the line of care from the central to the local level, to delineate a personalised pathway for each patient and to create effective changes in patients’ autonomy and daily lives.
