The “Roadmaps for up-scaling. Possible pathways to patient-centred, holistic care for RD Patients and patients with complex needs in Austria, Romania and Spain” were elaborated by the Centre for Social Innovation (ZSI) in the scope of their work on social policy innovation based structural reforms in the scope of the INNOVCare project.
